Report: Service Manager Introduction to the Model Housing Plus has been initiated as a customised model of high-level support to promoteindependent living in the community for people known as chronic homeless service users whose tenancies or home occupation are at risk because of ongoing challenges in relation to high-risk behaviours, addiction self-neglect, hoarding and/or incapacity to maintain their home to an adequate standard.
